No. It is an internal record-keeping aid, not the OSHA 300, 300A or 301 form, not a submission to any agency, and it does not reproduce their layout. Those are prescribed federal forms with their own rules — this helps you gather what they ask for so the responsible person can complete them.
Does it tell me if a case is recordable?
No, and it deliberately doesn't. Recordability, day counts, privacy-case handling, severe-event reporting deadlines and retention are set by the current recordkeeping rule and your state plan. The decision is the employer's; this form records that it was made and by whom.
