Studio Incident Log
What happened, when, who was involved, what was done at the time, who else was informed, the follow-up and the sign-off — covering reactions, injuries, sharps incidents, equipment failures and client complaints. Incidents are rare enough that nobody has a routine for recording them and common enough that every studio eventually needs one. Written at the time it is a record; written later it is a recollection, and the difference matters to an insurer.

Frequently asked
Should I notify my insurer?
Where your policy requires it. This tool states no timeframe because policies differ; check yours and record what you did.
Does it assess how serious something was?
No. It records what was observed and what was done. Severity is not a form's judgement to make.
