Which written aftercare instructions were given, in what form, when, that they were also explained verbally, who to contact with questions, and the client's acknowledgement. Note carefully what this is: it records that aftercare was provided. It does not contain aftercare instructions and does not generate them. Those are yours — they differ by service, by product and by practitioner, and a generic set printed by a tool is exactly the thing that should not be handed to a client.

Why doesn't it include the aftercare instructions?
Because they are not ours to write. They depend on the service, the products you used and your training. This records that yours were given, which is the part that gets disputed.
Is the client's signature necessary?
Your own practice and local requirements govern. A signature is what evidences that they received it, which is usually the question.
