Client Consultation Record
The record of the conversation before the work: the service discussed, the history the client discloses, what was agreed including placement and size and colour where that applies, and — the field most often left blank and most often needed — what you advised against and why. It records what the client stated and what was agreed. It is not a clinical assessment and it makes no judgement about whether a service suits anyone; that is your professional call and your local rules, not a form's.

Frequently asked
Does it tell me if a service is suitable for this client?
No. It records what the client disclosed and what you decided. Suitability is your professional judgement, informed by your training, your product instructions and your local requirements.
Why record what I advised against?
Because that is the conversation that gets disputed. A client who was told a design would not hold at that size, and chose it anyway, is a very different situation six months later if it was written down at the time.
