Grease Trap (FOG) Service Log
The on-site log your FOG program expects: device and size, hauler and permit number, manifest and volume removed, full pump-out rather than a skim, baffles and tees, measured solids and grease depths, inlet and outlet flow. Cleaning frequency and the pump-out criterion are entered from your sewer authority's rule. Keep it with the hauler manifests and hand it over when an inspector calls.

Frequently asked
Does this count as filing with the sewer authority?
No. It is a record-keeping form kept on site — not a certification, not a discharge permit and not a regulatory filing. The hauler's manifest remains the disposal record and must be retained with it.
How often does my trap need pumping?
That is set by your local sewer authority or pretreatment program and neighbouring towns often differ, so the frequency and the depth criterion that triggers a pump-out are fields you fill in, not constants we print.
