Music Use Licence
Licensing an existing work: who is licensing, to whom, which work, and — the field people forget — whether it covers the composition, the recording, or both. Those are separate rights, frequently owned by different people, and a licence covering only one of them is a licence to be sued by the other. Then the permitted use, media, territory, term, exclusivity, credit, fee, and what happens when it expires.

Frequently asked
Composition or recording?
Both exist for almost every track and they can be owned separately. If you are licensing a track someone recorded of a song someone else wrote, you likely need permission from both. The form makes you state which you hold.
Is this legal advice?
No. It records the deal. Music licensing differs by territory and this is an area where getting it wrong is expensive.
